Inland Valley Home Health Agency is the sister company of Affective Healthcare which specializes in the rehabilitation of individuals who are home bound. We provide skilled nursing care, home health aide services, and therapy services. Our professional field staff provides intermittent visits and hourly skilled care to our patients in the comfort of their own home.  Services are performed under the direction of a patient’s primary care physician.  Inland Valley Home Health Agency is licensed by the Department of Health and is Medicare Certified.

HOME HEALTH SERVICES

Home health services are for those recovering from a surgical procedure, illness or injury. Our goal is to help restore our patients health to an optimal level to regain their independence at home. Services can be provided in the place you call home. This can be a house, apartment, board and care facility, assisted living facility, or other home setting.

GENERAL INFORMATION

Who qualifies for Home Health Services?

Anyone under the care of physicians who would benefit from care in the home qualifies for home care. Inland Valley Home Health Agency services San Bernardino County, Riverside County, Los Angeles County and Orange County.


How is Inland Valley Home Health Agency reimbursed for its services?

Medicare, Medi-Cal, HMOs, PPOs, Veterans Administration, workers compensation, and regional centers provide coverage for our home health services. Personal payment is also an alternative.


How do I start receiving services?

To begin receiving home health care from Inland Valley Home Health Agency, you may call us directly at (800) 803-0072, or physician, hospital, clinic, social agency, family member, or friend may refer you. Once we have received your referral, a registered nurse will make an appointment with you at your earliest convenience to determine your qualifications for home health care. Once you qualify, we will work closely with your personal physician to develop your health care plan.

Inland Valley Home Health Agency’s Board of Directors and administrative staff are committed to the provision of home health care services that are guided by quality, risk management, and cost-effective services for the agency’s clients.
